Physicals and Immunizations
Just a reminder that physical and dental examinations are required for all Kindergarten and transfer students who are new to District 135. Students will be excluded from school if the physical has not been completed or if there are missing immunizations. If you still need to get the required immunizations for Kindergarten, or if you have been contacted regarding missing immunizations on you child's medical form please call your school nurse. She will give you information on clinics where you can receive free immunizations. Screenings for hearing and vision take place in the fall. Parents will be contacted if any problems are noted. Please call the nurse if you are having any health concerns regarding you child.
Food Allergies
Please note that all primary and intermediate schools in District 135 are following a new policy regarding food in the classroom. Please visit the food service page for details.
Administration of Medication
Medication can be administered only with a signed physician's order, which is part of the District 135 medication form, which can be obtained in the nurse's office. This policy is inclusive of both prescription and over the counter medications. Medications must be brought to school by the parent or guardian in the original, properly labeled container. Only the nurse or a school administrator can administer medicine to a student.
Health Records
By law, all children entering school for the first time and all sixth graders must present evidence of a recent physical examination and of immunizations against certain diseases before the first day of school. A dental examination is strongly recommended. These records must be turned into the school nurse's office by the beginning of the school year.
Illinois state law regulating physical exams has changed this past year. Physicals are now required for students entering 6th grade, instead of 5th grade. Since this is a transition year and many families had already submitted physicals for 5th grade the legislature has determined that those physicals will be accepted for 6th grade this coming year. Only those who did not submit a 5th grade physical must submit one for 6th grade. The Hepatitis B series is still required for 5th grade.
