Kindergarten Registration

To be enrolled in kindergarten for the 2008-09 school year, children must be five years old on or before September 1, 2008. Did you miss the kindergarten registration round-up in February?

New Student Registration
Please come to the Administration Center as soon as possible.
These items are necessary for school registration:

1. Proof of residency within Orland School District 135 boundaries from both Category 1 and Category 2 (as described below). The parent's name and address must appear on the documents and they must be current. Acceptable proof:

Category I (one document required)
- Most recent home property tax bill
- Home closing/mortgage papers
- Signed and dated lease with two rent receipts
- If parent is not the homeowner: notarized affidavits of residency from the resident owner and the resident custodial parent along with supporting documents from both Category 1 (owner) and Category 2 (parent)

Category II (three documents showing proper address are required)
- Driver's license
- Voter registration card
- Cable, phone, or credit card bill
- Public aid card
- Homeowner's insurance premium receipt
- Gas, electric, or water bill
- Vehicle registration

2. Transfer slip from the latest school showing school and grade attended. In IL, this is the Illinois State Transfer Form.

3. Name and address of the student's previous school. This is the full and complete address to which a records release form will be sent.

4. Student's birth certificate. The student's government-certified document, not a hospital commemorative card.

5. Check or money order for the 2008-09 student registration fees: $127 for kindergarten, $159 for grades 1-6, and $154 for grades 7-8 if paid after June 30, 2008.

6. Student's health records. A copy of the last physical exam including immunization dates should be provided. If a child is transferring from outside of Illinois, a new physical exam is required.

7. If a child has special needs, any records necessary for a special placement.

8. Any student custodial agreements, decrees, judgments, or other documents if applicable.
